Your Health Your Choice Respecting Choices Site Visit
The Your Health Your Choice Advance Care Planning (ACP) Project will host a Respecting Choices® three day site visit in Genesee County, Michigan, in preparation for ACP expansion efforts throughout the community.
The site visit will provide an opportunity for community ACP stakeholders to learn in more detail about the ACP model (day 1), training for 32 additional individuals to become ACP facilitators (day 2), and training for eight ACP instructors (day 3).
The Your Health Your Choice Respecting Choices® site visit will promote a community understanding of the ACP model and expand the availability of free advance care planning services at a variety of settings throughout the community. Please contact us if you are interested in more information.
